5. Method and Legal Basis of Personal Data Collection
Ваши персональные данные собираются через формы, заполняемые вами физически при обращении в нашу клинику, запросы на прием через наш веб-сайт, телефонные разговоры и переписку по электронной почте.
Ваши персональные данные обрабатываются на основе следующих правовых оснований, указанных в статьях 5 и 6 KVKK:
- Existence of your explicit consent
- Explicit provision in laws
- Necessity for the performance of the contract
- Fulfillment of legal obligation
- Protection of public health
6. Rights of the Data Subject
You have the following rights under Article 11 of KVKK:
- To learn whether your personal data is processed
- To request information if your personal data has been processed
- To learn the purpose of processing your personal data and whether it is used in accordance with its purpose
- To know the third parties to whom your personal data is transferred domestically or abroad
- To request correction if your personal data is incomplete or incorrectly processed
- To request deletion or destruction of your personal data under the conditions stipulated in KVKK
- To request notification of operations to third parties to whom your personal data is transferred
- To object to the emergence of a result against you through the analysis of processed data exclusively by automated systems
- To claim compensation for damages if you suffer damage due to unlawful processing of your personal data
